Download the form, print it out, fill it in , scan it and either...

• Email it as an attachment to treasurerclwa@gmail.com and etransfer your dues to the same email address.
  
  NOTE: IF YOU ETRANSFER YOUR DUES, DON'T FORGET TO EMAIL THE PASSWORD TO THE TREASURER SPEPARATELY.
